TWH is proud to be part of the pioneering Coordinate My Care service which simply and efficiently shares urgent care plans for residents between the Home and all urgent care service providers.
These urgent care providers include 111, out of hours GP,s as well as ambulance & emergency services.
The purpose of CMC is to ensure any urgent care to be received by our residents can be easily accessed and followed by any urgent care providers in the event of an emergency. This care includes: patients diagnosis, medical details, resuscitation status, medications and most importantly the resident's own wishes.
Joining the CMC service earlier this year was the initiative of our equally pioneering Registered Manager Ajitha who explains why she felt it was important for TWH to be part of CMC :
"CMC is a great way of keeping everyone is in the loop with the Resident in the middle. By sharing patient wishes at times of most need, the programme is helping people receive care in the place they would like, most usually the home. Our aim is to put every Resident's wishes as the utmost priority and help achieve them with great respect and dignity."
